Use of this form is voluntary; however, any club member submitting a request for payment needs to have the following information in writing for the Treasurer's records: Date and minutes when the request for GFDTC funds to be used was first proposed and approved by the membership if the request is over $100; Amount of funds approved; when the goods or services was or will be received; which program or committee were the funds for ESPECIALLY if property is involved.
